Valued Clients,

In order for us to appropriately respond to your needs, we must have the following documents to complete the intake process*:

  • Driver’s License or State Issued ID for all persons age 18 or over
  • Social Security Card for all occupants (example: children, spouse, or other dependents living with you)
  • Copy of entire, signed Lease Agreement and landlord’s contact information
  • Notice of unpaid rent/notice to vacate
  • Proof of employment/Income (two most recent pay stubs)
  • Benefits award letter(Social Security / VA / SNAPS if applicable)
  • Six months of bank statements
  • Additional documents supporting your need for assistance (example: unexpected medical bills/car repairs
  • If requesting utility assistance, most recent past due utility bill

*NOTE: An intake assessment does not guarantee services. Eligibility cannot be determined during intake. Your eligibility is determined by a Case Manager after completing the intake process. Eligibility determination can take 3 to 5 business days.
